This Notice explains how protected health information about you may be used and disclosed, and how you can get access to this information.

1. Purpose of this notice

This Notice describes how your protected health information (PHI) may be used and disclosed and how you can access it. Your clinical care and records are provided through our clinical partner, MD Integrations, and the licensed clinicians who treat you, who maintain your medical record.

2. How your health information may be used and disclosed

  • Treatment — to evaluate you, prescribe, and coordinate care with pharmacies and clinicians.
  • Payment — to bill and process payment for the services you receive.
  • Health care operations — for quality, safety, and administrative functions.
  • As required or permitted by law — including for public health and safety purposes.

3. Your rights regarding your health information

  • To access and obtain a copy of your medical record.
  • To request corrections to your record.
  • To request an accounting of certain disclosures.
  • To request restrictions or confidential communications.
  • To receive a paper copy of this Notice on request.

4. Our safeguards

PHI is stored and processed within a HIPAA-compliant environment by our clinical partner under appropriate business associate arrangements. Oxiline is designed so that PHI is not stored on our own servers.
